GRACE launches its community to help shape the future of cardiovascular care in Europe
Connect. Learn. Influence the future of cardiovascular care in Europe
How can cardiovascular care become more proactive, better connected and more responsive to patients’ needs? Answering this question requires the experience of people across healthcare, research, industry and policy, alongside those living with cardiovascular conditions.
The GRACE Community is an open, multi-stakeholder space bringing these perspectives together. Through dialogue, knowledge exchange and collaborative activities, it will create opportunities to explore new care models and support the development, validation and future adoption of innovations developed within the GRACE project.
We invite healthcare professionals, researchers, healthcare organisations, patients and advocacy groups, industry representatives and policy actors to join the conversation.
Bringing different perspectives together
The shift from reactive approaches towards more proactive cardiovascular care involves changes in how care is organised, delivered and experienced. Digital technologies offer opportunities to support this transition, but their relevance depends on how well they address clinical needs, patient expectations and the realities of healthcare systems.
The GRACE Community provides a space to explore these questions together. Healthcare professionals can bring insights from everyday practice; patients can share what matters to them throughout their care; researchers and innovators can discuss emerging approaches; and health system stakeholders can contribute perspectives on implementation and adoption.
By connecting these experiences, the community aims to encourage exchanges that can inform and refine innovative cardiovascular care models.
Who can join?
The community welcomes stakeholders from across the cardiovascular innovation ecosystem, including:
- Healthcare professionals and researchers interested in discussing innovative cardiovascular solutions and contributing to early validation and co-creation activities.
- Hospitals, healthcare organisations and service leaders exploring new care pathways and the practical challenges of introducing them into everyday care.
- Patients and advocacy groups willing to share experiences and expectations to help ensure that future care pathways remain patient-centred and relevant to real needs.
- Industry leaders, SMEs and innovation networks interested in exchanging perspectives on scalability, interoperability and emerging digital health models.
- Policymakers and health system stakeholders interested in the conditions that could support the future adoption of cardiovascular innovations.
Why take part?
Joining the GRACE Community offers an opportunity to connect with a diverse European network and learn from people approaching cardiovascular care from different perspectives.
Members can gain insights into emerging care pathways, contribute expertise to collaborative discussions and explore connections between healthcare and patient needs, research and innovation. The community also offers opportunities to share experiences and make their work more visible within the GRACE ecosystem.
For some, the starting point may be a clinical or research question. For others, it may be an experience of receiving care, an implementation challenge or an interest in finding potential collaborators. Each can contribute to a better understanding of what future cardiovascular care needs to deliver.
