A European mission to transform cardiovascular care: The GRACE Project Kicks Off
The GRACE Project officially began its journey in January 2025, marking the start of an ambitious European collaboration to transform cardiovascular care. A few months later, in March 2025, the consortium held its kick-off meeting at the Università Campus Bio-Medico di Roma, bringing together public and private partners, clinicians, researchers, patient associations, and industry leaders to chart a new course for cardiovascular innovation.
Cardiovascular diseases remain the leading cause of mortality globally, accounting for more than 18 million deaths every year. Despite remarkable progress in medicine, early detection, diagnosis, and care coordination still face major challenges. Fragmented healthcare pathways and the growing burden of chronic conditions place significant pressure on healthcare systems and professionals.
Co-funded by the Innovative Health Initiative (IHI), GRACE unites 27 partners from across Europe to address these challenges. Over its 54-month duration, the consortium will design, develop, and test scalable, AI-driven and data-supported solutions to improve clinical workflows, empower healthcare professionals, and enhance patient outcomes.
Throughout 2025, the project has been laying the groundwork for six real-world clinical pilots, each focusing on key cardiovascular conditions such as heart failure, atrial fibrillation, coronary artery disease, and stroke. By integrating digital tools, predictive analytics, and coordinated care models, GRACE seeks to bridge the gap between innovation and implementation — ensuring that new solutions deliver tangible benefits for patients and healthcare systems across Europe.
The kick-off meeting represented not just the official start of the project, but the beginning of a shared journey. Participants reaffirmed their commitment to collaboration, sustainability, and impact — shaping the future of cardiovascular medicine through innovation and partnership.
